We are looking for our partner a full-time or part-time General Practitioner/Geriatrician M/F Coordinating Physician on a permanent contract. You will work in 2 of its EHPADs (63 places) and (35 places) located in the Aude, Corbières Territory, 40 minutes from Narbonne and 45 minutes from Perpignan, so very close to the sea and the mountains! Don't hesitate any longer!! You will work in a multidisciplinary team within the elderly pathway of the Aude territory to support the dependent elderly person in order to maintain their autonomy, family and social ties and prevent the risks of complications linked to their vulnerability. You are in charge of developing and monitoring the establishment's care plan, the medical assessment of residents thanks to your gerontological skills and the management of the care team. Regarding the resident's gerontological assessment, you carry out several assessments (cognitive tests, nutritional assessment, etc.), the results of which allow the care team to organize interventions with the resident. You coordinate the interventions of the various stakeholders, you are in contact with external stakeholders (physiotherapist, speech therapist, etc.) as well as with treating physicians and specialists. You ensure, with the coordinating nurse of the EHPAD, that medical and paramedical records are properly maintained. You participate in the implementation of agreements with health establishments and gerontological networks or any other player in the geriatric care sector. You will be able to participate in the Territorial Resource Center project on the Corbières.
